The first document is necessary in order for your child to attned the Club.

The second document is necessary if you qualify for any type of DES assistance, food stamps, free and/or reduced lunch, as well as if you are unemployed.

The third document is necessary if you would like to apply for a volunteer position at the Club. A fingerprint card will also be necessary upon initial approval.
